Steps:
- After completing 15 minutes Free phone call, you will receive an email from Orchid (our HIPPA compliant electronic medical record [EMR] platform).
- You will make an Orchid account, enter your payment infromation, and confirm your visit.
- Once your appointment is confirmed, you will recieve office forms (HIPPA, Privacy, Intake) to complete prior to your visit with us.
- We strongly encourage you to read all the forms entirely before signing them. Electronic copies will be available for your referrence in your Orchid account.
- Please make a list of previous medications that you have taken for anxiety, ADHD, depression, OCD, sleep issues, psychosis or other difficulties.
- Additionally, please include medication dosage, duration, what worked, and any negative side effects experienced.
- Detailed family history of mental health and medications taken by biological family members.
- Tele-psychiatry (Online/Virtual) option is available. You can simply log into your Orchid account, clinic on your appointment in the calender to join videovisit.
- For best tele-psychiatry experience possible, please make sure to have a good internet connection and a device with audio-video capabilities.
- We strongly encourage you are at home or safe private place during video visit. Avoid being in driving car or unsafe place.
